received or will receive an Employee
Certification Form from the City of
Chicago Benefits Management
Division. This form refers to
dependent children who receive
medical benefits through the
member’s healthcare plan. Members
must complete and submit
this form for tax reporting purposes
by March 31, 2010. Members who fail
to complete and submit this form run
the risk of potentially having taxes
levied against them for receipt of
these benefits.
Members should note that this
process is one of the results of
Public Act 95-0958 with an effective
date of January 1, 2010. Dependent
children’s benefits are not affected
in any way by the completion or
submission of this form. Costs for
dependents currently covered will
also not be affected. Any questions
should be directed to the Benefits

PRIMARY POLITICAL
ENDORSEMENTS
01/05/10
The Political Action Committee met
on December 16, 2009 for the purpose
of interviewing candidates for
political offices in the Primary
Election set for February 2, 2010.
The recommendations are compliant
with the established procedures in
that only candidates who request the
endorsements were considered for
same. Candidates for statewide
offices were not considered per the
decision of the Illinois FOP State
Lodge to not make such endorsement
for these primaries. The Committee
made recommendations to the Board
based upon submitted materials and
the interview procedure. Those
recommendations were voted on by the
entire Board on January 5, 2010 and
that vote resulted in the following
endorsements. Please give
consideration to these Board
endorsements when you vote in the
primary.
