This is the application form for the annual scholarship drawing of Chicago Lodge #7. The scholarship awards are in the amount of $1,000.00 each and are given in honor of our late State Lodge President, Richard Lis.  There will be twenty names drawn at the November 16, 2022 General Meeting, and we will also draw five additional names, in case some of the winners are not in college or elect not to continue their education.
